This can develop into a condition called\u00a0<a href=\"https:\/\/www.mskcc.org\/cancer-care\/types\/esophageal\/risk-factors\/barrett-s-esophagus\">Barrett\u2019s Oesophagus<\/a>, which increases the risk of oesophageal cancer. Barrett\u2019s oesophagus should be closely monitored, and in some cases treated, to reduce this risk.<\/p>\n<\/div>\n<aside><\/aside>\n<\/aside>\n<p>Most people are familiar with heartburn and the discomfort it brings. But they may not be aware that it can be a sign of gastroesophageal reflux disease (GORD), a condition that can potentially lead to\u00a0<a href=\"https:\/\/www.mskcc.org\/cancer-care\/types\/esophageal\">oesophageal cancer<\/a>. GORD develops when stomach acid that digests food washes up into the oesophagus.<\/p>\n<p>After someone has had GORD for many years, it can advance to a condition called Barrett\u2019s Oesophagus. Oesophageal cells that have been bathed in acid for a long time begin to thicken as a defense mechanism to withstand the acid.<\/p>\n<p>\u201cThe oesophagus wants to protect itself, so the cells in the lining begin to change,\u201d says Memorial Sloan Kettering thoracic surgeon\u00a0<a href=\"https:\/\/www.mskcc.org\/cancer-care\/doctors\/daniela-molena\">Daniela Molena<\/a>. \u201cThe normal tissue lining the oesophagus begins resembling the lining of the stomach or intestine.\u201d<\/p>\n<p>Barrett\u2019s Oesophagus is considered a precancerous condition and increases oesophageal cancer risk. While only a small percentage of patients with Barrett\u2019s Oesophagus end up developing oesophageal cancer, it is important to monitor the condition in case it begins to progress.<\/p>\n<p>Dr. Molena explains how to know if you have Barrett\u2019s Oesophagus. In addition, she and MSK clinical dietitian-nutritionist Christina Stella describe what you can do to reduce the risk of it developing into cancer.<\/p>\n<h3 id=\"what-are-the-symptoms-of-barrett-s-esophagus\">What are the symptoms of Barrett\u2019s Oesophagus?<\/h3>\n<p>Barrett\u2019s Oesophagus can develop after long-standing GORD, so a person with the condition may be used to experiencing heartburn or regurgitation soon after eating. But as GORD progresses to Barrett\u2019s, sometimes these symptoms disappear, Dr. Molena says.<\/p>\n<p>\u201cOften when I ask patients if they have reflux or heartburn, they\u2019ll say, \u2018I used to have that when I was younger, but I don\u2019t anymore,\u2019\u201d she says. \u201cThe pain often goes away when Barrett\u2019s develops, which can be counterproductive because people seek less help for their symptoms.\u201d<\/p>\n<p>In addition, some people with Barrett\u2019s never have any symptoms. \u201cThat\u2019s why it can be hard to catch patients early and treat it before it becomes cancer,\u201d Dr. Molena says.<\/p>\n<h3 id=\"how-is-barrett-s-esophagus-diagnosed\">How is Barrett\u2019s Oesophagus diagnosed?<\/h3>\n<p>Doctors use a technique called endoscopy to diagnose and stage Barrett\u2019s Oesophagus. In this test, a gastroenterologist puts a thin, flexible tube with a light and a camera at the tip, called an endoscope, down the throat to look at the lining of the oesophagus.<\/p>\n<p>\u201cWith the endoscope, you can see the lining in the lower part of the oesophagus, which is where the changes usually can be seen,\u201d Dr. Molena says. \u201cThe lining will have a different colour. If it looks like Barrett\u2019s, we\u2019ll take a tissue sample for our pathologists to examine.\u201d<\/p>\n<p>The pathologist can determine how far along the Barrett\u2019s Oesophagus is and whether it has become precancerous or cancerous and has the potential to spread to other areas of the body. At MSK, specialized gastrointestinal pathologists review the tissue and determine whether it has no dysplasia (precancerous cells), mild or high-grade dysplasia, or if it is already cancerous.<\/p>\n<h3 id=\"what-can-i-do-to-prevent-or-reverse-barrett-s-esophagus-and-minimize-esophageal-cancer-risk\">What can I do to prevent or reverse Barrett\u2019s Oesophagus and minimise oesophageal cancer risk?<\/h3>\n<p>The most important step is to reduce and minimise acid reflux. Effective ways to do this include:<\/p>\n<div>\n<ul>\n<li>lose weight if you are overweight and obese<\/li>\n<li>don\u2019t smoke<\/li>\n<li>limit alcohol consumption<\/li>\n<li>eat smaller meals, and wait a few hours before going to bed<\/li>\n<li>avoid late-night snacking<\/li>\n<li>sleep with your head slightly elevated (nighttime GORD is more damaging because the oesophagus is also \u201csleeping\u201d and does not promptly push acid down)<\/li>\n<\/ul>\n<p>&#8220;Overall, focusing on more fruits and vegetable in the diet is the best approach to avoiding not just GORD and Barrett&#8217;s Oesophagus, but all forms of chronic disease&#8221; &#8211; Christina Stella, Clinical Dietitian-Nutritionist.<\/p>\n<p>MSK Senior Clinical Dietitian-Nutritionist Christina Stella, who counsels many people at risk for oesophageal cancer, says research has bolstered the strong link between obesity \u2014 especially around the belly \u2014 and Barrett\u2019s Oesophagus and oesophageal cancer. \u201cWorking with a dietitian and the primary care team on weight management is a very important lifestyle step that patients can take to reduce their risk,\u201d she says.<\/p>\n<p>Apart from this, changing your diet can make a big difference in preventing acid reflux. People often think reflux is tied to acidity in specific foods, such as citrus fruits or tomatoes, but that\u2019s a misconception. Ms. Stella says it\u2019s more important to follow a low-fat diet.<\/p>\n<p>\u201cThis includes limiting both the bad fats you find in fried food, red meats, and butter, and the fats usually considered \u2018good,\u2019 like those found in avocados, olive oils, and some nuts,\u201d she says. \u201cThe reason is that fat sits in the stomach for a long time and it can push up on the door into the oesophagus, which lets out some of the acids causing that burning feeling.\u201d<\/p>\n<p>\u201cOverall, focusing on more fruits and vegetable in the diet is the best approach to avoiding not just GORD and Barrett\u2019s Oesophagus, but all forms of chronic disease,\u201d she adds.<\/p>\n<p>Apart from minimising all types of fat, some people may want to try limiting coffee, mint, and chocolate, as they can be triggering for reflux, Ms. Stella says. Also, limiting alcohol may decrease gastric acidity, and alcohol consumption is a well-known risk factor for all cancers.<\/p>\n<p>Sticking to any kind of diet is challenging for most people. Ms. Stella says sometimes interim steps work best \u2014 for example, reducing the quantities of certain foods rather than trying to eliminate them altogether. \u201cIf someone comes and tells me they can\u2019t go without steak, my response would be, all right, can we do steak once a week instead of steak three times a week? That\u2019s going to be a lot better for their health.\u201d<\/p>\n<p>&#8220;Barrett&#8217;s Oesophagus progresses in a very methodical way, which is why we like to see patients through the whole process.&#8221; &#8211;\u00a0Daniela Molena, Thoracic Surgeon.<\/p>\n<h3 id=\"what-are-the-options-for-someone-diagnosed-with-barrett-s-esophagus\">What are the options for someone diagnosed with Barrett\u2019s Oesophagus?<\/h3>\n<p>Barrett\u2019s Oesophagus may not need treatment if it is diagnosed at an early or intermediate stage. Someone at low risk can be monitored with an endoscopy every three to five years to see if the cells change. \u201cBarrett\u2019s Oesophagus progresses in a very methodical way, which is why we like to see patients through the whole process,\u201d Dr. Molena explains.<\/p>\n<p>Usually, doctors can remove precancerous lesions using an endoscope before they become more serious. At MSK, they may destroy the lesions using endoscopic methods, including cryoablation (freezing), radiofrequency ablation (radio waves), or photodynamic therapy (destroying the abnormal cells using a light-sensitive drug and laser). They also can remove the lesions with a snare or dissecting knife if there is a concern for more advanced disease.<\/p>\n<p>Because Barrett\u2019s Oesophagus is related to GORD, it\u2019s very important that patients with Barrett\u2019s be treated for GORD with medications or procedures that alleviate the condition, even if they aren\u2019t experiencing symptoms.<\/p>\n<p>MSK has a dedicated program for monitoring and treating people with Barrett\u2019s Oesophagus.
